-reporting to the general practitioner / referrer and medical specialists;
-financial administration.

By signing the intake form, you allow Food Mind Body to use your personal data for the above purposes.
For the above objective(s), Food Mind Body can request personal information from you such as name, address, telephone number, e-mail address, date of birth, sex, BSN, health insurance policy number, general practitioner / referrer / specialist, health data.

Food Mind Body takes appropriate technical and organizational measures to protect personal data against unlawful processing.

Food Mind Body works together with partners, for example for the administration and invoicing of the treatments. It is therefore possible that we provide your personal details to those partners. Our partners are obliged to handle your personal data with care. They may not use your information for anything other than what they were provided to them.

Food Mind Body stores personal data and data on dietary treatment as long as required by law. You have the right to inspect your file. If you wish the file to be destroyed after completion of the treatment, you can request the dietician / practitioner for this.
